This is a practical book aimed at those whose daily task it is to think about and manage or regulate the risks of hazardous technologies. The book is not targeted at any one domain, but attempts to identify general tools and principles that are applicable to all organizations facing dangers of one sort or another. This could include banks and building societies just as much as nuclear power plants, oil exploration and production, chemical process plants, and air, sea and rail transport. The emphasis is placed upon the principles and practicalities of defenses against accidents, and how to meet the challenges and minimize risk. After reviewing the types of error and violation and the conditions that provoke them, the author sets out the broader picture, illustrated by examples of three system failures. Central to the book is a comprehensive review of error management, followed by chapters on: managing person, the task and the team; the workplace and the organization; and creating a safe culture. It is then rounded off and brought together, in such a way as to be readily applicable for those who can make it work, to achieve a greater and more consistent level of safety in maintenance activities. This title offers a withering and clear-eyed critique about (but not for) intellectuals that explores their impact on public opinion, policy, and society at large. It has not been by shaping the opinions or directing the actions of the holders of power that modern intellectuals have most influenced the course of events, but by shaping public opinion in ways that affect the actions of power holders in democratic societies, whether or not those power holders accept the general vision or the particular policies favored by intellectuals. In "Intellectuals and Society", Thomas Sowell not only examines the track record of intellectuals in the things they have advocated but also analyzes the incentives and constraints under which their views and visions have emerged. Ultimately, he shows how often intellectuals have been proved not only wrong, but grossly and disastrously wrong in their prescriptions for the ills of society. Modern technology has now reached a point where improved safety can only be achieved through a better understanding of human error mechanisms. In its treatment of major accidents, the book spans the disciplinary gulf between psychological theory and those concerned with maintaining the reliabiblity of hazardous technologies. Much of the theoretical structure is new and original, and of particular importance is the identification of cognitive processes common to a wide variety of error types. Using a light touch and sensible techniques, Why Don t We Listen Better? puts the experience of Jim Petersen s forty years of counseling and pastoral ministry at your service in improving your communication and relationships. A Talker-Listener Card process and specific listening techniques for instant use show you how to turn win-lose communication into come-alive cooperation. The purpose of this book is to explore the human contribution to both the reliability and resilience of complex well-defended systems. The predominant mode of treating this topic is to consider the human as a hazard, a system component whose unsafe acts are implicated in the majority of catastrophic breakdowns. But there is another perspective, one that has been relatively little studied in its own right, and that is the human as hero, a system element whose adaptations and compensations have brought troubled systems back from the brink of disaster on a significant number of occasions. What, if anything, did these heroes have in common? Can these abilities be 'bottled' and passed on to others?Insightful, eloquent and extremely accessible, James Reason provides the reader with an essential guide to human behaviour on individual and organisational levels, examining the human from both perspectives. Quantum Leadership: Advancing Innovation, Transforming Health Care, Third Edition provides leaders in the health care industry with the skills they need to ensure that their organizations are guided accurately and effectively through periods of transformation. As rapid changes continue to affect the health care system, this text offers strategies for handling challenges that arise in health care organizations to better assist leaders in creating a healing environment for both the providers and consumers of health care. The Third Edition has been completely revised and updated and contains two new chapters: Creating Context: Innovation as a Way of Life and Evidentiary Leadership: An Expanded Lens to Determine Healthcare Value.
