Indonesia s health system performs well with respect to some health outcomes and financial protection, but there is potential for significant improvement. High-level political decisions are necessary on key elements of the health financing reform package. The key transitional questions to get there include: the benefits that can be afforded and their impacts on health outcomes and financial protection; how the more than 50 percent of those currently without coverage will be insured; how to pay medical care providers to assure access, efficiency, and quality; developing a streamlined and efficient administrative structure; how to address the current supply constraints to assure availability of promised services; how to raise revenues to finance the system, including the program for the poor as well as currently uninsured groups that may require government subsidization such as the more than 60 million informal sector workers, the 85 percent of workers in firms of less than five employees, and the 70 percent of the population living in rural areas. An additional US$5 billion also came available from increasing revenues and declining debt service. 2 Despite the 2005 domestic fuel price adjustments, Indonesia still spends US$12 billion on subsidies annually, mainly on fuel and electricity. 3 Thirty-six percent of all public spending is now in the hands of sub-national governments. 4 While spending on education since the crisis has nearly doubled and spending on health has increased almost 70 percent, spending on infrastructure investment remains significantly less than pre-crisis levels (below 3.4 percent of GDP). 5 Indonesia spends about 50 percent of its total annual capital expenditure in the final quarter of the year. This is the only in-depth study of social policies in Southeast Asia. It compares social security, health and education policies in Indonesia, Malaysia, the Philippines, Singapore and Thailand. After describing the policies and assessing their adequacy and equity implications, it examines the forces that have shaped them. It concludes that social programs (except for primary education) in the region are both inadequate and inequitable. It argues that the reason for this is political rather than cultural or socio-economic.
